Appendix 2: List of Forms in CS Pertaining to Graduate Students

Application

  • Application for Admission or Readmission to Graduate Study and for Change of Major
    This form is accompanied by information and instructions for Graduate Application to the programs in CS.
  • Letter of Recommendation to The George Washington University
    This form is used to request a letter of recommendation for admission or for a fellowship at the George Washington University.
  • Request to Waive Admission Conditions for SEAS Graduate Students
    This form is used by a student's academic advisor to remove deficiency courses imposed at the time of admission.

Registration

  • Registration Transaction Form
    This form is used as the official advisor's approval form for telephone registration and for all drop, add, and withdrawal transactions.

Courses

  • Form 1
    The Form 1 (Graduate Program of Studies) is a list of courses which constitute a curriculum for a degree. It must be filled out by each student working toward a degree in CS, and approved by the advisor and the department. A student may not graduate until all courses listed on the Form 1 have been successfully completed. A Form 1 should be submitted during the first semester after matriculation; the contents may be revised at any time.
  • Request for Permission to Register for Undergraduate/Graduate Courses Outside of GW
    Generally speaking, permission to undertake a course outside GW is given for special reasons such as: course not offered at GW that semester and is required to continue studies; student will be located in an area outside of Metropolitan Washington, D.C. and wishes to continue studies; the course or instructor has a recognized outstanding reputation. A maximum of six credit hours may be transferred from another university toward a graduate degree.
  • Request for an Incomplete Grade
    An incomplete grade may be given for sufficient reasons, such as: severe illness, hospitalization, assignment by employee to duty which would delay completion of academic obligations, and death of close family member.
  • Application for Transfer of Graduate Credit
    A maximum of 6 credit hours may be transferred from another institution to a graduate degree program in CS.

Financial Aid

  • Application for Graduate Teaching Assistantship(GTA/GRA)
    This multi-page form must be filled out when applying for aposition in CS as a teaching assistant.
  • Application for Job as Grader
    The position of paper grader is distinct from that of a GraduateTeaching Assistant. Paper graders usually grade homeworksubmissions of students in class. Pay is on an hourly basis. Those interested in applying should fill out this form each semester.
  • Application for Job as Proctor
    A proctor monitors the taking of exams; it is an hourly job usually of short duration. A list of potential proctors is kept eachsemester in the CS office. Those interested in being a proctorshould fill out this form each semester.
  • Timeslips
    All positions paying an hourly wage require the submission of atimeslip accounting for the hours worked.

Computers

  • SEAS Computing Facility Account Request: Student
    A form available at the SEAS Computing Facility for requesting a student account for one semester on the SEAS computers. A computer access fee is charged.
  • SEAS Computing Facility Account Request: Class
    A form available at the SEAS Computing Facility for requesting a class account for use by students enrolled in classes in SEAS. The account is valid for one semester.
  • SEAS Computing Facility Account Request: Sponsored Research
    A form available at the SEAS Computing Facility for requesting an account for use in sponsored research.
  • SEAS Computing Facility Account Request: Departmental Research
    A form for an account which is available at the SEAS Computing Facility for direct collaborative research with a faculty member.
  • Student Account Request: George Washington University Computer Center
    A form available at the CIRC/us for requesting a student account.

Tuition Benefit

  • Application for Tuition Benefits
    A form for university employees to request tuition benefits. The form must be filled out prior to registration.

  • Procedures for D.Sc. Qualifying Examination in Computer Science
    Not a form, but an explanation of the procedures for the qualifying examination in the area of concentration of Computer Science.
  • Announcement: Final Examination for the degree of Doctor of Science
    A form which is submitted to provide information for a formal announcement of the dissertation defense. This form must be submitted at least six weeks prior to the dissertation defense.

Information Concerning Theses and Dissertation

Not a form, but a two-page set of instructions useful in the preparation of theses and dissertations at the George Washington University.

  • Form 2: Master's Thesis Adviser Form
    A form which must be filled out to state that the thesis option of the Master's degree has been chosen, with a research advisor selected.
  • Form 3: Master's Thesis Title
    A form for registering for the defense of the Master's thesis.
  • Master's Thesis and Doctoral Dissertation Submission Form
    A form which submits a thesis or dissertation to the CS department upon completion of the thesis.
  • Thesis and Dissertation Transmittal Sheet
    A checklist of places to send the completed thesis or dissertation. These include the CS department, the library, and University Microfilms.
  • Transmittal Sheet for Survey of Earned Doctorates
    A form available from CS and supplied by the National Research Council. This form must be submitted to NRC in order to provide a national database of research.
  • Publishing your dissertation through the University Microfilms International Dissertation Publishing Program
    A brochure from University Microfilms which describe the publication of the dissertation. Submission to University Microfilms is mandatory.
  • Application -- 5: Doctoral Candidates -- Permission to Register for Dissertation Credits
    After completion of coursework for the D.Sc. degree, a student must enroll for 24 credit hours in dissertation research. This form should be filled out for all students prior to registering for dissertation research.

Graduation

  • Application for Graduation
    A form which must be completed at the beginning of the semester in which a student expects to graduate (deadlines are February 1 for spring graduation, and October 1 for fall graduation).
  • Late Application for Graduation Petition Form
    A form which is submitted if the above form is submitted too late.

Other

  • Change of Academic Advisor Form
    This form, entitled "Memorandum to the Chairman of CS -- Subject: Change of Advisor" is used to select another academic advisor.
